Wound Care Software for Skilled Nursing Facilities
The SNF setting is the primary market for mobile wound care specialists. But "wound care software for SNFs" means different things depending on whether you are a contracted independent specialist or an employee of the facility. The billing model determines the software requirement.
Two Billing Models, Two Software Requirements
Model 1 — SNF Staff Nurse (PDGM Billing)
The SNF employs the wound care nurse. The facility bills wound care as part of the home health or SNF episode under PDGM. The facility's EHR (PointClickCare, MatrixCare, American HealthTech) handles all wound documentation and billing.
The wound care nurse uses whatever platform the facility already has. No separate wound care EMR needed. The limitation: OASIS wound documentation is staging-focused for payment grouping, not treatment-focused clinical billing. It does not generate Part B claims.
Model 2 — Contracted Independent Specialist (Part B Billing)
An NP or PA contracts with the SNF and sees wound patients as an independent provider. The specialist bills Medicare Part B under their own NPI using wound care CPT codes (97597, 11042, etc.). The SNF bills the episode separately.
Two independent billing transactions on the same patient visit. The SNF's PointClickCare does not handle the specialist's Part B billing. The specialist needs a complete, separate wound care platform.
SNF-Specific Requirements for Independent Specialists
Offline documentation: SNF wireless is unreliable. Enterprise IT controls network access. Platforms that require active connectivity fail in this environment. Non-negotiable.
Route-based multi-patient workflow: You are seeing 4–8 patients in sequence in the same building. The workflow must support rapid transitions — pull up next patient, document, move. The platform should not require logging out and back in between patients.
OASIS-supportive language: Independent specialists who document wound stage, dimensions, tissue composition, and periwound condition in OASIS-compatible format give the SNF MDS coordinator documentation they can reference directly on assessment days. This is a relationship differentiator that generates repeat referrals.
Per-patient healing trajectory: Measurement history across the full episode, trend visualization, area reduction percentage calculation for advanced therapy criteria.

The PointClickCare Confusion
The most common misconception in SNF wound care: "The facility uses PointClickCare so I should use it too."
PointClickCare is a facility EHR designed for SNF operations and PDGM billing. It does not generate Part B CPT-coded claims for independent providers. It does not have the LCD compliance logic needed for wound care billing. And as an independent contractor, you almost certainly do not have access to the facility's PointClickCare environment.
You need your own platform. The facility uses theirs.
